Login your Username and Password, under Work, select e-Form,<\/span><\/p>\n<p><span style=\"color: #000000\">Undergraduate Program Application for Transfer of Major (Degree Program) -1st Choice or 2nd Choice, or 3rd Choice.<\/span><\/p>\n<p><span style=\"color: #000000\">(The e-form will be available on November 20, 2025)<\/span><\/p>\n<p><span style=\"color: #000000\">2.<strong>\u00a0Department Transfer Examination: November 28 to December 2, 2025<\/strong><\/span><\/p>\n<p><span style=\"color: #000000\">3. In the e-form, please attached the Ming Chuan University Department Consultation Record for Department Transfer Application Form (Download from the Academic Affairs Division\/ Forms) and cumulative transcript. The hardcopy of the cumulative transcript need to be submit to the department that you want to transfer to. Those e-form applications that did not attached the Ming Chuan University Department Consultation Record for Department Transfer Application Form and the cumulative transcript will not be accepted.<\/span><\/p>\n<p><span style=\"color: #000000\">4.
